Junior Counselor Application Form – Addendum to Camper Application Form for 10th Grade Junior Counselors. First, enroll with the Camper Application, above. Then this application form is to be completed by the camper and submitted with or after parents have submitted the Camper Application Form.

Health History Annual Form – Required each year, completed and signed by a parent or guardian
Health Examination Record for Campers and Staff – Proof of health exam within 36 months, other forms acceptable. Forms should be uploaded to your CampInTouch account, or faxed to 860-456-2444.
